Summary: This article explains common ACH payment issue codes and what they mean. When a payment fails or a Notification of Change (NOC) is received, MineralTree Support will contact you with the details you need to correct the issue before sending more payments.
Overview
ACH payments can fail for several reasons, such as incorrect bank account information, authorization problems, or updates made by the receiving bank. When this happens, the ACH network sends a code that explains why the payment failed. MineralTree uses these codes to help you understand what needs to be fixed so future payments can go through.
There are two types of ACH issue codes:
- NOC Codes (Notifications of Change): Tell you that the receiving bank has updated account information that must be corrected before sending more payments.
- Return Codes: Tell you that a payment was rejected and did not go through.
MineralTree Support monitors both types and will reach out when action is needed.
ACH NOC Codes
A Notification of Change (NOC) is sent when the receiving bank updates information about a vendor’s bank account. For example, the vendor may have changed their account number, or the bank may have updated their routing number.
When MineralTree receives a NOC, Support will notify you so you can update the vendor’s bank details. Making these updates helps prevent future payment failures.
NOC Codes and Descriptions

| C01 | Incorrect bank account number | Bank account number is incorrect or formatted incorrectly. |
| C02 | Incorrect transit/routing number | Previously valid routing number must be updated. |
| C03 | Incorrect transit/routing number and bank account number | Routing number change requires an update to the account number structure. |
| C04 | Bank account name change | Customer name has changed or was submitted incorrectly. |
| C06 | Incorrect bank account number and transit code | Account number must be updated and payment code must reflect correct account type. |
| C07 | Incorrect routing number, account number, and payment code | All three fields require correction. |
ACH Return Codes
ACH return codes explain why a payment was rejected by the receiving bank. These codes help you understand what went wrong and what may need to be corrected before trying the payment again.
MineralTree Support will notify you when a return occurs and provide the return code.
Return Codes and Descriptions
| Code | Reason | Description |
|---|---|---|
| R01 | Insufficient Funds | The account didn’t have enough available funds to cover the debit. |
| R02 | Account Closed | The account was previously open but is now closed. |
| R03 | No Account / Unable to Locate Account | The account number does not match any account at the bank. |
| R06 | ODFI Requested Return | The originating bank requested the return. |
| R07 | Customer Revoked Authorization | The customer revoked authorization for the debit. |
| R08 | Payment Stopped | The customer placed a stop payment on the transaction. |
| R09 | Uncollected Funds | Funds exist but are not yet available (e.g., pending deposits). |
| R12 | Account Sold to Another DFI | The account has been transferred to a different bank. |
| R13 | Invalid ACH Routing Number | Routing number is invalid or inactive. |
| R16 | Account Frozen / Returned per OFAC | Account is frozen or restricted due to legal or OFAC requirements. |
| R20 | Non‑Transaction Account | Account cannot accept ACH transactions. |
| R23 | Receiver Refused Credit | Receiver rejected the credit entry. |
| R29 | Not Authorized by Corporate Customer | Corporate customer states the debit was not authorized. |
| R42 | Routing Number / Check Digit Error | Routing number or check digit is incorrect. |
| R43 | Invalid DFI Account Number | Account number is invalid for the receiving bank. |
